HELP Grind a shift 1 st &reverse

I cant get the clutch cable to adjust and work (cables loose) at 138 mm loke Haynes says ? (why)
the clutch isnt wore to the limit yet of 8.5 mm
the clutch lever is in the right spot (kind of).It grind when i want to go 1 st or reverse ... is something wore out such as pressure plate or shifter bushings .. Do i have to take some time and adjust everthing. I have to mash the pedal to the floor.. clutch is close to wear limits but not there yet.. changed the oil in tranny its a 5 sp 016z i really think its the clutch any 1 hace these problems before .

Do you mean a dead shift into either gear....I.E. when you are at a stop and shifting from neutral to reverse or 1st?
If yes simple solution. Shift into 2nd gear first and then sift into the respective gear that you want.

This will stop the grinding. Due to the way the syncros work.
